Golden Pathways Senior Care & Solutions provides chronic care management, remote patient monitoring when appropriate, virtual support, and care coordination for eligible patients.
02
Does Golden Pathways replace my primary care provider?
No. Golden Pathways works alongside your primary care provider, specialists, home health team, pharmacy, caregivers, and other treating professionals.
03
What is Chronic Care Management?
CCM provides ongoing support between traditional visits for eligible patients with multiple chronic conditions. It may include a personalized care plan, health and medication follow-up, coordination, education, and communication with the care team.
04
What is Remote Patient Monitoring?
RPM uses an appropriate connected medical device to securely transmit certain health measurements from home so relevant information can be reviewed and used to support the patient's care.
05
Will someone be watching my RPM device all the time?
No. RPM is not continuous emergency surveillance and is not an emergency-response service. Patients should call 911 or seek emergency care for an emergency rather than waiting for a device reading to be reviewed.
06
Can my caregiver or family member be involved?
Yes. With appropriate patient authorization, Golden Pathways can include trusted family members or caregivers in communication and care coordination.
07
What happens after a hospitalization?
Golden Pathways may help review important discharge information, medication changes, follow-up needs, monitoring, and communication with the appropriate members of the patient's care team.
08
Does Medicare cover these services?
Medicare Part B covers eligible chronic care management and remote patient monitoring services when applicable requirements are met. Deductibles, coinsurance, or other cost-sharing may apply depending on the patient's coverage. Patients should contact Golden Pathways or their insurance plan with coverage questions.
09
Can I stop participating?
Participation is voluntary. Service-specific Medicare, insurance, billing, or equipment-return requirements may apply when services are discontinued.
